Muscogee (Creek) Nation School Clothing Program

The 2010-2011 application will be available beginning May 3, and ending September 3, 2010.

The School Clothing Program is funded by the Muscogee (Creek) Nation to assist eligible Creek students.  The program will provide students a grant of $200 to purchase school clothing items and/or foot apparel.  Eligible Creek students must be 3 years old and enrolled in a certified Head-Start; at least 4 years old and enrolled in a public/private school pre-k program, no higher than the 12th grade.  Home Schooled children ages 5 to 18 years old an participating in an accredited curriculum are also eligible.  Grants will be provided in the form of a check, payable to the head of household listed on the application, and will be awarded once per school year.  All completed applications will be accepted on a first come, first serve basis until funds are exhausted or September 3, 2010, whichever comes first.

Application & Award Process: 

• The application must be filled out completely.  The application is also available in person, by fax, or by mail at the Muscogee (Creek) Nation School Clothing Program building located at 2971 N. Wood Drive, Okmulgee, Ok, 74447.  Phone (918) 756-2024.

Applications will also be available at any MCN Indian Community Centers and MCN Child Development Centers.

• Verification of school enrollment. A school enrollment form will be attached to the application for all children in Head Start through the 12th grade.  The school enrollment form must be completed by a school official.
  
•  Verification of home school (if applicable) must accompany the application. Verification of home school is:  1) Accredited curriculum OR original receipts of textbooks purchased.  AND 2) The signing of the attached Affidavit by the individual providing the education.
  
•  The custodial parent must complete the application.  If the applicant is not the custodial parent, documentation in the form of a Legal Custody Order or Guardianship Order through a court must accompany the application.  Notarized statements are NOT accepted.
• A copy of the student's Muscogee (Creek) Nation citizenship card must accompany the application.  CDIB cards will not be accepted.
  
• If you have three or more eligible children, you are required to fill out the attached W-9.  At the end of the year, a 1099 form will be mailed to you in accordance with IRS regulations.  Please wait to file your taxes until you receive this form.
  
• Checks will be delivered by mail through the U.S. Postal Service.
  
• All applicants should allow 6-10 weeks for processing.

• Grants are to be used for the purchase of clothing ONLY.
• Examples of non-clothing items include, but are not limited to, backpacks, school supplies, pajamas, robes, sleep pants, groceries, makeup, jewelry, perfume, hair products, tobacco, alcohol, fuel, food, check cashing fee's, etc.
• Documentation of clothing purchases is required.  All original receipts must be returned to the School Clothing Program by mail or in person within 90 calendar days of the check issuance date.
• Students who are 18 and attending a post-secondary educational institution (college or vo-tech) are not eligible for this program.
• The Muscogee (Creek) Nation and the School Clothing Program are absolved from any further responsibility for checks upon delivery to the grantee.
